The Emperor of All Maladies: A Biography of Cancer, by Siddhartha Mukherjee

The Emperor of All Maladies is a magnificent, profoundly humane “biography” of cancer—from its first documented appearances thousands of years ago through the epic battles in the twentieth century to cure, control, and conquer it to a radical new understanding of its essence.

Physician, researcher, and award-winning science writer, Siddhartha Mukherjee examines cancer with a cellular biologist’s precision, a historian’s perspective, and a biographer’s passion. The result is an astonishingly lucid and eloquent chronicle of a disease humans have lived with—and perished from—for more than five thousand years.

The story of cancer is a story of human ingenuity, resilience, and perseverance, but also of hubris, paternalism, and misperception. Mukherjee recounts centuries of discoveries, setbacks, victories, and deaths, told through the eyes of his predecessors and peers, training their wits against an infinitely resourceful adversary that, just three decades ago, was thought to be easily vanquished in an all-out “war against cancer.”

The book reads like a literary thriller with cancer as the protagonist. From the Persian Queen Atossa, whose Greek slave cut off her malignant breast, to the nineteenth-century recipients of primitive radiation and chemotherapy to Mukherjee’s own leukemia patient, Carla, The Emperor of All Maladies is about the people who have soldiered through fiercely demanding regimens in order to survive—and to increase our understanding of this iconic disease.

Riveting, urgent, and surprising, The Emperor of All Maladies provides a fascinating glimpse into the future of cancer treatments. It is an illuminating book that provides hope and clarity to those seeking to demystify cancer.

Amazon.com Review
Amazon Best Books of the Month, November 2010: "In 2010, about six hundred thousand Americans, and more than 7 million humans around the world, will die of cancer." With this sobering statistic, physician and researcher Siddhartha Mukherjee begins his comprehensive and eloquent "biography" of one of the most virulent diseases of our time. An exhaustive account of cancer's origins, The Emperor of All Maladies illustrates how modern treatments--multi-pronged chemotherapy, radiation, and surgery, as well as preventative care--came into existence thanks to a century's worth of research, trials, and small, essential breakthroughs around the globe. While The Emperor of All Maladies is rich with the science and history behind the fight against cancer, it is also a meditation on illness, medical ethics, and the complex, intertwining lives of doctors and patients. Mukherjee's profound compassion--for cancer patients, their families, as well as the oncologists who, all too often, can offer little hope--makes this book a very human history of an elusive and complicated disease. --Lynette Mong

From Publishers Weekly
Starred Review. Mukherjee's debut book is a sweeping epic of obsession, brilliant researchers, dramatic new treatments, euphoric success and tragic failure, and the relentless battle by scientists and patients alike against an equally relentless, wily, and elusive enemy. From the first chemotherapy developed from textile dyes to the possibilities emerging from our understanding of cancer cells, Mukherjee shapes a massive amount of history into a coherent story with a roller-coaster trajectory: the discovery of a new treatment--surgery, radiation, chemotherapy--followed by the notion that if a little is good, more must be better, ending in disfiguring radical mastectomy and multidrug chemo so toxic the treatment ended up being almost worse than the disease. The first part of the book is driven by the obsession of Sidney Farber and philanthropist Mary Lasker to find a unitary cure for all cancers. (Farber developed the first successful chemotherapy for childhood leukemia.) The last and most exciting part is driven by the race of brilliant, maverick scientists to understand how cells become cancerous. Each new discovery was small, but as Mukherjee, a Columbia professor of medicine, writes, "Incremental advances can add up to transformative changes." Mukherjee's formidable intelligence and compassion produce a stunning account of the effort to disrobe the "emperor of maladies." (Nov.) (c)

From Booklist
*Starred Review* Apparently researching, treating, and teaching about cancer isn’t enough of a challenge for Columbia University cancer specialist Mukherjee. He was also moved to write a biography of a disease whose name, for millennia, could not be uttered. The eminently readable result is a weighty tale of an enigma that has remained outside the grasp of both the people who endeavored to know it and those who would prefer never to have become acquainted with it. An unauthorized biography told through the voices of people who have lived, toiled, and, yes, died under cancer’s inexorable watch. Mukherjee recounts cancer’s first known literary reference—hence its birth, so to speak—in the teachings of the Egyptian physician Imhotep in the twenty-fifth century BCE, in which it is clear that Imhotep possessed no tools with which to treat what appears to be breast cancer. His cryptic note under “Therapy:” “There is none.” Throughout cancer’s subsequent years, many more physicians and scientists with names both familiar and obscure attempted and occasionally succeeded in deciphering or unlocking keys to many of the disease’s mysteries. Alas, this is not a posthumous biography, but it is nonetheless a surprisingly accessible and encouraging narrative. --Donna Chavez

OFF THE CHARTS
By Kenneth E. MacWilliams
You remember the scene in the film "Butch Cassidy and the Sundance Kid"? From the top of the bluff looking into the distance at dusk, Butch sees the lights of the pursuing posse which doesn't stop tracking them even at night and says "How many are following us? They're beginning to get on my nerves. Who are those guys?" In the same threatening way cancers have been dogging human beings since the dawn of time, and although we now know quite a lot about cancer we still don't really know "who are those guys" or how to shake them. And they sure are "beginning to get on our nerves" as Butch said. Almost one out of four of us will eventually wrestle with cancer -- the defining illness of our generation -- and lose our lives in the process. Until it catches up with us most of us will try to ignore this fact, just as when we were very young children alone in our bedroom trying to go to sleep at night we tried to ignore the monster that we sometimes feared might be lurking in our bedroom closet.

Enter oncologist Siddhartha Mukherjee who almost parentally takes us by the hand to give us the courage to open with him the door to that dark and foreboding closet in order to see what is really lurking inside. Since eventually most of us are going to have to wrestle with this monster anyway -- either as a victim or as a loved one of a victim -- looking intelligently and closely into that dark closet does diminish fear and enhance wise perspective. And on this incredible journey into the depths of that darkness, what an absolutely marvelous guide is this modern day Virgil called Siddharta Mukherjee as he leads us on this long and often harrowing journey through the swarth that cancer has cut through mankind throughout time.

Mukherjee is a veritable kaleidoscope. Turn his writing one way and you experience him as an exciting writer of page-turning detective stories or mystery stories; turn him another and he's a highly effective communicator of cellular biology; turn him a third and you get superb science writing; turn him a fourth and he has the grandeur and broad sweep of an excellent historian. It's hard to believe that this one book, combining all of these appealing characteristics, is the work of just one man. And underlying it all is his sterling medical training and credentials which have been enumerated often elsewhere.

The book itself is a tour de force. It is the first book of such extraordinary scope regarding cancer. Its architectural structure brings to mind Melville's Moby Dick and how effectively and artfully Melville braided together the three strands of his great classic: a grand adventure story, the technology of whaling, and a treatise of humanity and philosophy. Equally effectively does Mukherjee weave together all the various facets of this iconic disease throughout history, from describing cancer from the patient's perspective, to viewing the never ending battles of physicians and medical researchers with cancer over the centuries, to examining the mysteries of the cellular nature of cancer itself and what really goes on in there, to the pro and con impact of this never ending plague on the spirit of the individual human and on our race as a whole, to peering into a crystal ball for a glance of cancer's and our future together. While doing all of this the alchemy of Mukherjee's writing continually turns science into poetry and poetry into science.

Simply put, it is so good, and so incandescently clear and lucid, and so powerful, and so engrossing, and so easily consumed that you will not lay it down without someone or circumstances forcing you to.

Had I read this book in my teens I would have found my life's career. I can only imagine that while you are reading this book, somewhere there will be some very young teenage girl or boy who will also be reading it at the same time you are, and who will become totally hooked by this book just as you will be, and who will go on to make a career in cancer research, a career that might provide the breakthrough that humanity has been searching and hoping for all of these many centuries. Thus although you will never know it, you will have "been there" at the initial motivation of that person and thus indirectly present at the earliest genesis of the eventual great idea.

This book has THAT potential. It is THAT good.

read it after your treatment
By Aaron McLoughlin
Last April I was diagnosed with acute leukaemia. After a stem cell transplant I am coming up for a year.

When you are told you have cancer you are bewildered. You are also very angry. I asked myself was there something I had done in my past that was going to deprive me seeing my two sons grow up into happy young men and dads. The first two weeks go by in a weird nightmare. Day 17 your hair falls out. Your peeing orange from the chemo drugs, which have put me off lucozade for life. You double check all your insurances are up to date and update a well to make sure my wife does not have any hassles with the tax authorities. At the age of 44 you are very angry. You realise you are likely going to die. You are angry because you have no idea what is doing it. What you planned for when you were older is all meaningless. But, thanks to certain stubbornness and amazing treatment and care, and a generous sift of life from a German donor of life giving stem cells, I am alive.

This book helps explain many of the questions I had. It does it in a way that makes sense if you don't have a degree in science. What was until recently a death sentence is no longer the case. The battle against cancer was waged by intrepid individuals, and this book explains the war so far. It outlines the causes of cancer, whether it is a virus, bacteria, induced by smoking or chemicals, or just our own body playing up and turning on itself. It explains how our own understanding is still basic but advancing year by year, and treatments, if not cures, are being found for many, although not all cancers.

I learned that was once a death sentence is not the case today. I am looking forward to see my sons become men. This book gave me clarity, it gave me hope.

An insider's view of cancer
By Ionut Trestian
I only read this book because Ken Burns produced a documentary on it that is coming out in the Spring of 2015. I really love Ken Burns documentaries, hence the interest. Across the book, there is only one common character, cancer. Although cancer is not a single disease but a collection of several diseases characterized by uncontrolled growth and spread of cells in the body, the book portrays cancer as a great villain, lurking in the shadows, ready to strike at any time. What makes this story different and far from dry is the way S. Mukherjee tells it: "the story of leukemia - the story of cancer - isn't the story of doctors who struggle and survive, moving from one institution to another. It is the story of patients who struggle and survive, moving from one embankment of illness to another. Resilience, inventiveness, and survivorship — qualities often ascribed to great physicians — are reflected qualities, emanating first from those who struggle with illness and only then mirrored by those who treat them. If the history of medicine is told through the stories of doctors, it is because their contributions stand in place of the more substantive heroism of their patients."

Across the book, we are also introduced to ways of fighting or stalling the advance of cancer: radical surgery and radical mastectomy, X-rays, cytotoxics, monoclonal antibodies, tyrosine kinase inhibitors and S. Mukherjee explains really well how all of the above function (or don't function in some cases). One of the strengths of the book is that it gives a behind the scenes look at how certain drugs or procedures came to be (Druker's struggles with developing imatinib) or how other procedures were proven to be too radical and changed such as Halsted's radical mastectomy.

The fight to find a cure for cancer has triggered enormous social forces in the 20th century and in the book we are introduced to some of the main characters: Sidney Farber and the Jimmy Fund, Mary Lasker and the American Cancer Society both determined to enact policy changes that will get more resources allocated to the war against cancer. These are just a few figures in this war, but there were other forces as well that fought for cigarette labeling for example, or more personal struggles related to compassionate drug use.

S. Mukherjee ends the book on a more positive note. All throughout the book we get the impression that primitive forces are battling a very complex disease, using disfiguring surgery or drugs that oftentimes end up causing cancer themselves. The final few chapters are not so gloomy, he takes a molecular biologist's view of the disease and explains our current understanding of the processes and pathways involved and you do get the impression that by 2050 we will be able to target the specific pathways and mutations that make up a particular form of cancer.

Excellent anatomy reference book even though it's quite pricey
By Parka
[[VIDEOID:7eee2848a7131e4cd204901c51348bdc]]The concept of this book came from the other co-author Uldis Zarins, a classically trained sculptor. While he's learning sculpting, he was frustrated by the lack of anatomy books for sculptors, and so he decided to create such a book. The book was crowd-funded successfully on Kickstarter in May 2013.

So now the book is out.

Anatomy for Sculptors is available in paperback, hardcover and PDF. On Amazon, there's also the Kindle edition but the book's split and sold as separate titles namely for Figure & Torso, Head and Neck, Upper Limb and Lower Limb. I don't have the Kindle edition but from the preview pages available on Amazon, the layout is similar to the physical book, which means the text are going to be too small on a 6-inch Kindle. The book is in full colour so you don't want to read it on any black-&-white Kindles either.

This is a fantastic anatomy reference book, not just for sculptors.

There are lots of wonderful pictures included such as photos of live models with muscle diagrams overlaid, 3D scans, skeletons and schematic drawings. And of course there are diagrams of proportions for bodies and head sizes. The coverage is comprehensive, from head to toe.

Photos of the live models are particularly useful. They are usually shot from various angles. Then the photos are overlaid with simple schematic drawings showing the muscle groups, and then it's further overlaid with more detailed muscle fibre drawings. All these are in full colour and with the multiple angles and overlays, you can really get a good idea of the form and structure of the muscles.

The main difference between this book versus other anatomy books is the inclusion of the various body postures.

In addition to the usual standing figures, there are many examples of stretched muscles that come from different body postures such as twisting torso, arms moving from the top, front of chest, side, and to the back, behind the head and hand on shoulder. This is helpful because you don't have to second guess how any muscle will look like when in stretched mode or moving into different positions. Other anatomy books mostly show just the front, sides and back of the body and limbs, with the muscles in relaxed state.

Most pictures in the book are self explanatory. The text are mostly labels, captions and tips on things to take note of. The few pages on facial expressions would be more helpful with explanations though. Anyway for facial expression, check out The Artist's Complete Guide to Facial Expression.

Overall, this can be considered an indispensable anatomy reference book for artists and sculptors.

It's certainly an expensive book. Whether it's worth it depends on you. You may complain about the price, but I doubt you will complain about the quality.

To save some money, you might want to get the PDF version which is about half the price of the paperback.

As to why the book is so pricey, Kondrats said that it's because they had to purchase licensed anatomical photographs, hire a photographer, models, editor and graphic designer. And all that took them 5 years to produce the book. Coming up with a book like this is not easy, and the amount of effort they put in is evident in the production quality and end product.

(See more pictures of the book on my blog. Just visit my Amazon profile for the link.)

Image series with 3d in mind - and that's how anatomy should be explained!
By Teno Petri
Have to say this book is good! You have to pay quite a lot for it, but not for nothing. Quite the opposite. The subject matter and how things are presented here is first class. All unnecessary is stripped away. Almost every single page of this book is full of essential visually presented information for artistic use. There is no vague academic explanations of medical details that no artist will read.

The single greatest thing in the book is obviously the images. They are not just pictures or drawings of a sudden body parts. Actually images are series of images which explain each other. These series of images tries to explain the subject matter in 3 dimensional concepts.

I'm afraid this book will make many of your older anatomy books obsolete. That is fine, If you have the money pay the hefty price.

The best of the Great Britain travel books that I have read!
By S. Johnson
In planning a trip to Great Britain, I have purchased three travel books: Lonely Planet, Eyewitness and Frommer's. I would rank them in that order as far as helpfulness in planning our trip. Here are my reasons: I have found Lonely Planet's sections on "where to sleep" and "where to eat" are much more extensive, informative and include a wider variety in price ranges than the other two books. Also, they have more information on some of the smaller towns, that are off the beaten track. I enjoyed the wonderful photographs, maps and illustrations in the Eyewitness Guide to Great Britain and if you can afford two books, it is a great way to prepare for what you will see. However, since I only have room for one travel book in my small suitcase, I plan to take the Lonely Planet book with me on our upcoming trip.

Excellent, but many poor maps
By Peter from Socorro, NM
This is an excellent guidebook. The only problem with it is the quality of the city maps, which are almost impossible to read. I saw an earlier edition in which they were in color and much better. They must have tried to save money in this edition by doing them in grayscale.

PREFACE

Never has the field of money, banking, and financial markets been more exciting. Financial innovation, deregulation, and geopolitical considerations have produced sweeping changes in the international financial landscape. By its very nature this is a big picture topic: an exploration of the institutions and markets that define the global financial system and how economic policy making influences these institutions and markets. A distinguishing feature of Principles of Money, Banking, and Financial Markets is making sophisticated concepts such as asset pricing, financial contracting, and rational expectations accessible to undergraduates. The tenth edition continues a tradition that readers have come to expect – a focus on modern analytical perspectives presented in a casual, conversational style.

New to this Edition

We have made a number of key changes in this tenth edition, beginning with Chapter 1. In addition to providing an overview of the entire text, the first chapter links the field of money, banking, and financial markets to specific careers so that readers can see the connection between the topics covered and life after graduation. Reflecting the dramatic changes that have recently become part of our daily news diet, we have introduced a new emphasis on the consolidation of the financial services industry. While this emphasis may be seen in many chapters, it is most evident in substantially revised sections of Chapter 11, The Nature of Financial Institutions, and Chapter 15, The Regulation of Markets and Institutions. New developments in global markets, including the Asian financial crisis and recent developments in the European Union (Chapter 10, Understanding Foreign Exchange), are also addressed in this edition. The recent revamping of the Federal Reserve's FOMC announcements has simplified the discussion of Fed policy in Chapter 22. Finally, as we discuss in detail below, new and exciting supplementary materials have been developed for the tenth edition.

Organization

Part I briefly introduces the role of money, markets, and institutions within the framework of the overall economy. Part II launches an intensive examination of financial instruments and markets, including an analysis of the level and structure of interest rates (Chapters 4 and 5), the pricing of risky assets (Chapter 7), and an overview of derivatives (Chapter 9) and foreign exchange (Chapter 10). We then turn in Part III to a discussion of financial institutions, including how asymmetric information explains the fundamental nature of financial intermediation (Chapter 11).

The focus in Part II on markets and in Part III on intermediaries sets the stage for our unique analysis of financial system architecture in Part IV. It begins with an analysis of how financial contracts address the information problems presented by small and large businesses (Chapter 14). This analysis explains why small firms are dependent on financial intermediaries for external financing while large firms have access to the publicly traded securities markets. This discussion sets up another distinguishing feature of our text: an examination of the regulatory structure of the United States that compares and contrasts the regulation of financial intermediaries and the regulation of the securities markets (Chapter 15). We then turn to an international comparison of financial system design that takes into account alternative regulatory structures (Chapter 16). Japan and Germany illustrate how intermediary-dominated systems work, while the United States and United Kingdom represent how markets-dominated systems function. We next look at financial innovation and how it can change a financial system (Chapter 17).

In the final two parts of the book, we return to the traditional macroeconomic framework as we consider the art of central banking (Part V) and monetary theory (Part VI). We have updated all of these chapters. The last chapter of the book (Chapter 30) ties everything together by showing how to combine observations on institutions, markets, and the overall economy to understand current financial sector developments, including the ups and downs of the stock and bond markets.

Exactly how you organize this course depends on what you want to accomplish. With this in mind, we have written the chapters so that for the most part they are self-contained, thereby permitting maximum flexibility. Nevertheless, the ordering of chapters reflects the fact that most instructors now teach a financial-markets and institutions-oriented course, with central banking as the backdrop. Thus, there are good reasons for following the book chapter by chapter, with omissions reflecting your own particular emphasis. For example, it is possible to omit Chapters 6 through 8 to avoid capital market theory and performance and to drop the discussion of derivatives in Chapter 9 without sacrificing continuity. On the other hand, Chapter 10 on foreign exchange should appear early in the course because international discussions are interspersed throughout the text. Similarly, for a course focused on monetary theory and policy, it is possible to jump to Part V (the art of central banking) and Part VI (monetary theory and policy) right after completing Part I and Chapters 4, 5, and 10 from Part II.

Pedagogical Aids

The text is augmented by a number of pedagogical aids that expand upon and clarify important topics. The chapters are liberally spiced with extracts from current articles in the financial press that highlight key issues. These appear as In the News boxes. Sometimes we like to take a deeper look at a topic mentioned in the text. We do so in a separate section labeled Off the Record. Sometimes we just can't contain ourselves and feel compelled to offer our own opinion in boxes called Going Out on a Limb. Finally, we offer straightforward, no-nonsense help in interpreting financial charts and tables in our Reading the Financial News boxes.

Each chapter concludes with a summary recapping the key points. A list of key terms follows the summary to help students with the language of money, banking, and financial markets. And we also provide a set of questions for each chapter that students can use to test their grasp of the material.

Supplements to Accompany Principles of Money, Banking, and Financial Markets

The tenth addition is accompanied by a wide array of supplementary materials. These items are available to qualified domestic adopters but in some cases may not be available to international adopters.

Print Supplements: The Instructor's Manual, by Fred C. Graham of the American University, contains teaching tips, sample essay and discussion questions, and answers to all of the end-of-chapter text questions. A Study Guide, also prepared by Fred C. Graham, includes chapter synopses, a review of the central themes, problems, essay questions, true-false, and multiple-choice questions. The Test Bank by Andrew J. Dane of Angelo State University consists of multiple-choice questions that are sorted by difficulty.

Technology Supplements: The tenth edition comes with two powerful new teaching tools: an Instructor's Resource CD-ROM and an Online Course Companion Web site. Fully compatible with the Windows 95 platform and Macintosh computers, the Instructor's Resource CD-ROM contains PowerPoint slides of all the figures and tables, word processing files for the entire contents of the Instructor's Manual, and Computerized Test Bank files. The Computerized Test Bank software (TestGen-EQ with QuizMaster-EQ for Windows) is a valuable test-preparation tool that allows professors to view, edit, and add questions. The text's Online Course Companion Web site is available at http://www.awlonline.com/ritter. This site offers multiple-choice quizzes as well as numerous Internet links for each chapter. An online syllabus builder allows instructors to create a calendar of assignments for each class. Users of the Online Course Companion Plus system will be able to track student activity with an electronic gradebook (please see your sales representative for details). Addison Wesley Longman's Product Support Web site, located at http://techsupport.awl.com, provides media product information and technical help for all these technology-oriented supplements. In addition to information about how to contact product support staff, the site includes product descriptions, answers to frequently asked questions, hardware requirements, Y2K status, and software downloads.

no math
By Scott
Overall this book was OK. It presented the key information in money and banking and the authors often made it entertaining to read. However the one major flaw with the book is the lack of ANY math. I know that some books are marketed for a more mathematical approach and thus contain more advanced mathematics accordingly. However this book had so little math it was laughable. I recall one section where the authors said "this can be proven mathematically, but for now just take our word for it..."

NO, I won't take your word for it, because knowing the underlying principals is to truly understanding anything.

Acceptable Book
By Suzuki Hitori
The book arrived in the condition that I had ordered it to be - used. There was some physical damage to the book but it was still very much useable. It saved me quite a bit of money so the condition is to be expected. Essentially you get what you pay for.
